Your Patient Portal is a secure, easy-to-use online platform that gives you 24/7 access to your personal health information and medical records. With just a few clicks, you can manage appointments, view test results, message your provider, and more from anywhere.

How to Get Started

Once you receive your login details:

• Click the Patient Portal link provided.
• Select “Patient Login.”
• Enter your username and temporary password.
• You’ll be prompted to change your password the first time you log in.

View Test Results

To check your lab or pathology results:

• Click on the Tests & Results tab.
• Select the date of the test to open your report.
• Click the information icon to see more details about your diagnosis.

If you have any questions or concerns about your results, you can message your provider directly from the portal.

Message Your Provider or Request Refills

Need to contact your provider or ask for a medication refill? Go to the Messages tab and click “Compose Message.” You can send a secure message with questions or refill requests anytime.

Sign Consent Forms

Need to complete your consent forms? Request access and a secure link will be sent to you through eDerm.

Access Your Medical Records

To receive your medical records, please request a secure access link. A link will be sent to you via ModMed (EMA).

Pay Your Bill

To make a payment, simply click Pay My Bill or access this payment portal via our website menu.
